Flu Shot Information for Families!

Southern Families, flu shot information and permission forms are being sent home! If you would like your child to receive a flu shot at school, please carefully review the information and complete ALL sections of the permission form.

Important Reminder: Completed and signed forms must be returned by the deadline, September 17th. Students without a completed and signed permission form will NOT receive a flu shot.

Please be sure to include a copy of your Insurance or Medicaid card as required.

Southern Public Schools flu shot clinic flyer. The flyer announces that the Beatrice Community Hospital Immunization Clinic will be at the school on October 7th, 2026, to provide flu shots. Parents are instructed to complete and sign the permission form and, if they have insurance, return a copy of the insurance card. All completed forms and insurance cards must be returned to school by September 17th, 2026. The flyer states that students will not receive a flu shot if the completed form and insurance card are not returned by the deadline because extra vaccines will not be available. Questions can be directed to school nurse Dyan Allington at dallington@southernschools.org or 402-645-3359. The flyer uses Southern Public Schools’ red, black, and white colors with medical illustrations of a syringe, bandage, calendar, and check marks.
Flu Shot Parent Letter – Image Description A white, one-page letter from the BCHHC Immunization Clinic at Beatrice Community Hospital explaining the school flu vaccination program. The letter tells families that only the injectable seasonal flu vaccine will be offered. To have a child vaccinated, parents or caregivers must complete and sign the permission form, answer all screening questions, and return it to the school two weeks before Flu Vaccine Day. Children without a completed and signed consent form will not receive the vaccine. Families with insurance or Medicaid are instructed to provide a copy of their insurance card. The letter also provides information about possible booster doses, insurance billing, and contact information for the BCHHC Immunization Clinic.
Flu Vaccine Permission Form – Image Description A white, one-page BCHHC Immunization Clinic Vaccine Screening/Permission Form from Beatrice Community Hospital. The form includes spaces for the student’s name, date of birth, sex, address, phone number, insurance information, school, and doctor. Parents or guardians are asked to answer 10 health screening questions with Yes or No, including questions about illness, allergies, previous flu vaccines, medical conditions, medications, and pregnancy. The form states that the vaccine will not be given if the form is not completed, signed, and returned to the school two weeks before Flu Vaccine Day. The bottom includes vaccine information and spaces for the parent or legal guardian’s signature, printed name, date of birth, and relationship to the student.

JAG juniors and seniors had a great opportunity to learn about the housing process with Krista from Brick and Mortar Realty and Amanda from Heartland Flats! Students explored the home-buying process, apartment renting, and careers in both industries. The best part was getting to tour both a house and an apartment!

Students walked away with some great takeaways, including the importance of credit, what to look for when making housing decisions, and why it’s so important to read through contracts before signing. A big thank you to Krista and Amanda for making this a hands-on, real-world learning experience for our students!

💜🧡 Big Give Kids is September 10! Thanks to Pinnacle Bank, students 18 and under can choose a participating nonprofit to receive a $10 donation—no money needed from students!

Southern is using this as a hands-on opportunity to teach students about giving back to their community. Students can complete their form at school, and SPS will transport students to Heritage Inn throughout the day to turn in their forms.
