Letter from the School Nurse and Medical Forms

Parents,
I would like to introduce myself: I am Marlys Koch, school nurse. I work one day each week here at Southern and divide my time between the high school and the elementary. Wednesday is the day I can be reached at school.
The following forms are to be filled out for each student at Southern Schools. The forms must be turned in to the school at the start of the 2008/2009 school year.
Only those students with asthma are required to fill out the asthma form.
The medication form needs to be filled out for each medication your child takes at school whether it is a routine or as needed medication.
The student information form is a form that is now required to be filled out each year for each student. You may copy this form or pick up additional ones for your children at the school offices or from the school website.
The immunization form will be required to be filled out for each student this year and will be updated whenever a student has additional immunizations. This form has all the new vaccines available and is similar to the Gage County Immunization Clinic form.
