Physical Form - Medical History Form

All Athletes must meet the following requirements before they start practice:
Return a medical physical card to the athletic office. All Athletic forms can be picked up in the High School Office.
Return to the respective office your physical form which contains guidelines giving the following:
- Parental/Guardian permission to participate in the sport.
- Parental/Guardian acceptance to Athletic Guidelines.
- Athletes acceptance to the Athletic Guidelines
- Parental/Guardian signature regarding athletic insurance.
